accepted as a student of the New Conservatory of Dallas (NCD) and
a participant in the Conservatory Music in the Mountains 2010 session,
I agree to abide by the regulations of NCD. I agree not to bring
any alcoholic beverages on the premises of Fort Lewis College in
Durango, CO, or to possess or to engage in the unauthorized use
of drugs or controlled substances while a student of NCD. NCD reserves
the right to immediately expel any student for violations of its
rules and regulations without refunding any part of the tuition
of fees paid. Consent is hereby given for the applicant, while a
student with NCD, to participate in NCD sanctioned programs without
compensation and for photographs/video/audio captured by NCD to
be used in any NCD materials. I understand that all balances must
be paid in full by June 10. |

set up to prevent spam and nonpaying entries. Should former problems
